Where the leg starts to open is the whole difference, and it changes what the rest of the outfit has to do. A leg that widens early reads long and gradual; one that holds close through the thigh and then breaks below the knee puts all of its volume into the length that follows, so the same hem measurement can look calm or theatrical depending on how narrow the leg is above it. Keeping the top half close is what stops the line from going shapeless.

What is the difference between flared and bell-bottom pants?

It is where the widening starts. A flare opens gradually from around the knee, so the line lengthens smoothly from thigh to hem. A bell-bottom stays close through the thigh and knee, then widens sharply below it, concentrating all the volume in the last stretch of leg. Bell-bottoms therefore look more dramatic at the same hem measurement, because the contrast with the narrow knee is greater. Both finish wider at the hem than a bootcut does.

How long should flared pants be?

Longer than you would hem a straight leg. Flares are designed to be worn with some height under the heel, so try them on in the shoes you intend to pair them with before marking the length. The back hem should sit close to the floor, roughly half an inch off the ground, with the front slightly shorter. Hem them too short and the flare loses the line it depends on.
